Our facility is located at 7900 MS Hwy 570 W (Smithdale Rd.)  just inside Amite County on the left at the Thompson Road intersection.  Check out the map.

Monday, Tuesday, Wednesday & Thursday: 7:30am - 5:00pm

Friday: 7:30am - 12:00pm

Appointments are available by calling us at the number below.  Work-in visits are also possible when you can't wait for an appointment.  We want to serve you the best that we can.

• At your next appointment, ask about online reporting of your lab results to your internet-based health record account.

• For refills of chronic medications within one business day, contact your pharmacist and ask  them to send in the refill requests electronically.  It's quicker, saves you from calling the clinic, and saves paperwork.
